What to Bring
When you come in for a screening or prepare for your screening by phone, you will want to have some specific information on hand. The following items will help ensure the accuracy of your screening and, therefore, of your results:
- Income information
- Social Security number
- Information about dependents, including Social Security numbers
Sign up for a Screening
Your screening can be done face-to-face or over the phone, and will take approximately 15 to 20 minutes. BESO screenings are conducted at CAP's main office at 4606 S. Garnett Road, Suite 100.
