Apply now for the 2010-2011 school year

Applications accepted Monday – Thursday, 7 a.m. – 6 p.m., at CAP’s main office, 4606 S. Garnett Road, in the Exchange Center East Building, First Floor. Paperwork during enrollment must be completed by a legal guardian.

 

To complete a child’s application, parents or guardians must bring:

  • Birth Certificate or Record of Birth (If child is four by September 1, 2010, a certified birth certificate is required.)

  • Current Shot Records

  • Proof of Address

  • Most Recent Tax Return, if applicable — a tax return is required if family had earned income; however, if tax return does not reflect family’s current income situation, proof of current income or unemployment should accompany application.

  • Proof of Any Public Assistance, if applicable

Incomplete applications will not be accepted. You can significantly increase the chance that your child will be enrolled by including a copy of your child’s most recent well-child check up.
