Apply now for the 2010-2011 school year
Applications accepted Monday – Thursday, 7 a.m. – 6 p.m., at CAP’s main office, 4606 S. Garnett Road, in the Exchange Center East Building, First Floor. Paperwork during enrollment must be completed by a legal guardian.
To complete a child’s application, parents or guardians must bring:
Birth Certificate or Record of Birth (If child is four by September 1, 2010, a certified birth certificate is required.)

Proof of Address

Most Recent Tax Return, if applicable — a tax return is required if family had earned income; however, if tax return does not reflect family’s current income situation, proof of current income or unemployment should accompany application.

Proof of Any Public Assistance, if applicable
Incomplete applications will not be accepted. You can significantly increase the chance that your child will be enrolled by including a copy of your child’s most recent well-child check up.
